Spironolactone

Spironolactone is a an effective, long-term systemic treatment used off-label to manage persistent hormonal acne in adult women. Originally developed as a diuretic (water tablet) and blood pressure medication, it has proven to be a highly effective and safe treatment for hormonal acne.

Hormonal acne is notoriously difficult to treat with topical treatments because the root cause lies in hormonal disturbances. Flare-ups, typically presenting as deep, painful, under-the-skin cysts around the mouth, jawline, and neck, are triggered by androgens (male hormones like testosterone).

Spironolactone addresses this root cause directly. It acts as an androgen receptor antagonist. When taken orally, it binds to the androgen receptors located on the sebaceous (oil) glands in the skin. By physically blocking these receptors, spironolactone prevents the body's natural testosterone from stimulating the glands. The result is a dramatic, sustained drop in facial oil production, shrinking of the pores, and the cessation of deep hormonal cysts.

Because it blocks male hormones, spironolactone is only prescribed to female patients. It is typically used at low dose (50-100mg daily). It usually takes 1-2 months of continuous use to see maximum skin clearance.

Common side effects include low blood pressure, mild dizziness or lightheadedness, breast tenderness and period irregularities. Because it causes the kidneys to retain potassium, caution is required with foods high in potassium and blood tests are usually required when starting the medication. However high potassium levels are extremely rare in otherwise healthy women.

Spironolactone is strictly contraindicated during pregnancy due to risk of fetal malformations. Patients must use highly reliable contraception while on this medication.
