Topical Metronidazole

Metronidazole represents one of the most longstanding and commonly used topical therapies for managing rosacea. Available by prescription in a variety of formulations including creams, water-based gels, and lotions (e.g. brand names like Rozex or MetroGel), it is a common first-line and maintenance therapy for mild to moderate papulopustular rosacea.

Although metronidazole is an antibiotic, it has strong anti-inflammatory and antioxidant properties. Rosacea skin is constantly under assault by free radicals and hyperactive inflammatory pathways that lead to redness and the eruption of pustules. Metronidazole acts to reverse this.

Metronidazole is very well tolerated, rarely causing stinging or irritation. This makes it an ideal candidate for long-term, daily maintenance therapy, helping patients keep flare-ups at bay over periods of years without damaging the skin barrier.

While metronidazole is good for treating the inflammatory bumps and pustules, in my experience topical Ivermectin is often more effective. Additionally, it cannot reverse diffuse background redness, nor can it remove prominent blood vessels.
